Forms and Documents

All returning players, current VHS students and incoming Freshmen interested in playing football for VHS during the 2017 season are asked to complete the online player interest form. This information will be used to update our email distribution list, order swag pack, generate team rosters, etc.

2017 Sport Physicals

Dr. Mark Pinsky and Staff will be offering $20 Sports Physicals for all VHS Football Players on Thursday, April 20th beginning at 5:30pm in the PT Professionals location 8045 Spyglass Hill Rd., Suite 103, Melbourne, FL 32940. You may now reserve you spot by filling out the  online sign-up sheet . Please remember that you do still have the option to get a physical from your own family doctor or any doctor that you choose.

3. All players must register on RegisterMyAthlete.com . Bring the physical form with you to your appointment.

*Remember, you MUST have a current Sport Physical on file with the school before being allowed to participate in any football practices.
2017  Jacksonville University Team Camp 
The Hawks will be attending Jacksonville University’s full contact padded team camp. The cost of the camp includes overnight accommodations and all of the players’ meals. This camp will be for Varsity team members. *Please note: Incoming Freshmen and JV players are welcome to attend if they would like to travel with the team however, they will not be guaranteed any reps during the football scrimmages.

Parents can send in weekly, bi-weekly or monthly payments if so desired however, full payment must be received on or before Friday, June 9th. Checks can be turned in to Coach Mays or mailed to the HAWKS QB CLUB.  

The Hawks Quarterback Club will be paying for a chartered bus to take the players and coaches to and from the camp. All players MUST ride the bus with the team. This is a MANDATORY Varsity team event.

​Please fill out the JU Medical Form and turn in by Monday, July 10th at 9am. If JU Medical Form is not turned in by then, players will not be able to participate in the team camp.
